Office Policies

Insurance Coverage Information

We submit insurance claims as a courtesy to you.  All co-pays and non-covered services are due at the time of the appointment.  All benefits quoted are not a guarantee of payment by your insurance company and final determination can only be made when the claim is processed.

Vision Insurance plans cover routine vision exams along with eyeglasses and contact lenses.  Vision plans only cover a routine eye exam.  They do not cover diagnosis, management, or treatment of eye diseases.

Medical Insurance must be used if you have any eye health problem or systemic health problem that has ocular complications.  Your doctor will determine if these conditions apply to you, but some are determined by your case history.

Refraction

Refraction is a test to determine how well your vision can be corrected and your overall visual function.  Even though it is an important part of any eye exam Medicare and most medical insurances, do not cover this service.  Your current refraction fee is $37.00.

Eyewear

We require a 50% deposit on all orders.  The remainder of the balance must be paid before you may take orders out of the office.

Providing Your Own Frames

We will be happy to make prescription lenses if you choose to bring in your own frames.  Due to the age and wear of an existing frame they may crack or break when new lenses are inserted, therefore we cannot be responsible for any damages or loss to the frame as they are being reused at your request.  Regardless of condition or age of the frame, this disclosure applies to any existing frames being reused at your request.

Frames

If your frames break under normal wearing conditions, we will repair or replace them free of charge for two years from the date of purchase.  A $50.00 restocking fee will be charged for any exchanges within 90 days.  We do not cover loss, theft or abuse.

Lens Warranty

Our lens treatments provide the most reliable and durable surface protection available.  However, any lens can scratch or break.  Please follow recommended procedures for care and cleaning.  Our warranty covers replacement of lenses when you purchase scratch resistant or anti-reflective coating.  With our warranty, your lenses can be replaced due to scratches or coating defects twice within two years of the original date of purchase.  Lens replacement must be the original prescription.  Our warranty does not cover hairline scratches which have no effect on vision.

Contact Lens Policy/Evaluation and Fitting Fees

When a contact lens prescription is needed, additional services and fees are required that are not part of the routine eye exam.  Contact lens evaluations must be performed within 60 days of the routine eye exam.  Contact lens prescriptions expire one year from the date of the finalized prescription.  Your eyes go through gradual changes in size, shape and physiological requirements (such as oxygen) over time, which can change the contact lens prescription and affect the health of your eyes.  Our fees vary on complexity and type the contact lens.  The fee you paid covers all disposable trial lenses and contact lens follow-ups for a 30 day period from the day of the initial exam.  If a medical visit is necessary within that period, the medical visit will not be included in the trial period, and will be filed with your medical insurance if available or subject to cash charges.  If for any reason the follow-ups go beyond the 30 day trial period, additional fees may be incurred.

Unopened, not expired and unmarked contacts that are in their original boxing may be returned for credit or exchanged.  These policies do not apply when purchasing your contact lenses elsewhere.

Pick Up Policy

Eyewear not claimed within 60 days will be returned to the manufacturer.  Contact lenses not claimed after 90 days will be returned to the manufacturer.  We will notify you upon the arrival of your order via telephone, we will notify you again 3 weeks later via postcard.

Occasionally when a lab delivers to us eyeglasses that do not meet our quality standards, they are returned for re-processing and the wait time until your receive your new glasses might exceed the original anticipated date.  We will make every effort to keep you informed as to the status of your order.

Cancellation Policy

All orders are a custom process and involve fixed costs to our practice.  All cancelled orders will be charged a $75.00 fee.

Shipping Charges

Should you wish for your order to be shipped to you additional shipping charges will apply.  We will ship your order using USPS Priority Delivery for domestic orders (paid for by the customer).  We cannot be responsible for any damage to frame/lenses/contact lenses during the shipment process. Contact USPS if you have any problems with the shipment.

Missed Appointments

If you cannot keep your appointment, please inform us at least 24 hours in advance.  Failure to do so will result in a $25.00 missed appointment fee.
